Family history of genetic condition

• You may be aware that a relative of yours has a genetic condition.  This may be confirmed based on genetic test results, or suspected based on their health problems. In order to provide you with the best assessment, we aim to gather information about the relative, with their permission.  This may involve you providing us with copies of relevant test results or doctor’s letters that your relative has passed on to you.  In other cases, we ask you to pass on a consent form (to release medical information) to your relative with the condition.  If your relative is comfortable to sign this and return it to us, we can then access relevant information directly from their doctors.  Their health information is crucial to provide you with an accurate assessment.

• We understand that sometimes detailed information may not be available from your relatives.  Once we have as much information as we can gather, we will arrange an appointment for you.  Sometimes gathering this information takes some time.
